Arkansas
Request through the Arkansas Department of Finance and Administration Driver Services website.
California
Request online or visit in person with a completed INF1125 form.
Colorado
Request a Motor Vehicle Record online through the Colorado DMV.
Connecticut
Request online or visit your local Connecticut DMV office.
Delaware
Follow the instructions on the Delaware DMV website for requesting your driving record.

Request a certified copy through the Georgia DDS website or visit a local office.

Complete IR002 form and submit by postal mail. Check “10-year history” and “Original issue date requested.” Page 3 requires a notarized signature.
New Hampshire
Complete Form DSMV 505. Section I: Box A. Section III: “License Applications and Letters of Verification.” Section IV: fifth box (employer/insurer).
New Jersey
Request a driving history abstract through the NJ MVC. Note: only certain locations provide abstracts in person.
New Mexico
Request through the New Mexico MVD website or visit a local office.
New York
Request a driving record abstract through the NY DMV or visit a local office.
North Carolina
Request through the NC DOT DMV website or visit a local office.
Oklahoma
Request a certified copy through the Oklahoma DPS or visit a local office.
